How to Order Veterinary Medications
1. Ordering prescription medications for your pet from us for the first time

Locate the items you would like to order for your pet on our website using the
search field above. If you can't find what you are looking for, call us
toll-free at 1-877-530-0799 and we'll try our best help you get the products
you need.
Once you have located the products you would like to purchase, fax or mail us:
- the completed Veterinary Medical Questionnaire
- the completed Veterinary Medication Order Form
- a copy of your pet's veterinarian prescription
- a copy of the pet owner's government issued photo identification

Please note: We can only process orders for veterinary medications by fax or mail with our veterinary medication forms.

RETURNING CUSTOMERS WHO HAVE PREVIOUSLY PROVIDED US WITH THEIR PET'S MEDICAL
QUESTIONNAIRE AND THEIR OWN IDENTIFICATION do NOT need to provide these to
us ever again as we keep these on file. However, please provide us with an
updated Veterinary Medical Questionnaire if there have been changes to your
pet's health status since the last time you provided us with this document.
RETURNING CUSTOMERS WHO ARE ORDERING PRESCRIPTION VETERINARY MEDICATIONS
WHICH HAVE REFILLS REMAINING ON THE PRESCRIPTIONS PREVIOUSLY SENT TO US are
only required to fax or mail us the completed Veterinary Medication Order
Form. You do NOT need to provide us with your pet's veterinarian
prescriptions as we already have these on file.
